Illinois Substance Abuse Testing Policy
POLICY IMPLEMENTATION CHECK LIST
For Illinois Facilities
(Includes both legally required and/or best practice items)
______ 1. Written Policy has been developed per state law and/or court cases.
______ 2. Notice of the substance abuse testing policy has been posted in conspicuous places throughout the work site.
______ 3. A copy of this policy has been given to each current employee.
______ 4. A copy of this policy is available for review by any employee in the office of _______________________.
______ 5. Where appropriate, the policy and all related documents have been translated into appropriate languages.
______ 6. Where applicable, all Union Representatives have been notified and an opportunity to negotiate has been given.
______ 7. A notice of our EAP and how to access it has been posted. (if applicable)
______ 8. A substance abuse professional resource file is being maintained in the office of ________________. (if no EAP exists).
______ 9. We are prepared to provide employees with written notice of any positive test results along with details of that employee's rights of appeal.
______ 10. We will pay all costs of any test required by this policy.
______ 11. Only those test methods allowed by applicable state or federal law will be utilized.
______ 11. We will warn an employee who tests positive that s/he is responsible for the cost of split specimen/re-analysis testing.
______ 13. We will provide employees who test positive with an opportunity to confidentially explain any result.
______ 15. We will provide all employees with appropriate training regarding this program.
______ 16. We will provide warning to all employees that the use of over-the-counter medicines obtained in foreign countries could cause a positive result.
______ 17. All tests will be conducted during normal, "on-duty" time.
______ 18. All pre-employment testing will be conducted BEFORE the candidate begins work.
______ 19. We will NOT require/request that employees disclose any prescriptions they are taking. They will be encouraged to discuss all medications with the MRO.
